Siamak Nabili, M.D., MPH, QME

Dr. Siamak Nabili is a dual board-certified physician in Internal Medicine and Hospice & Palliative Care with more than 20 years of clinical, leadership, and medical review experience. His primary area of expertise is Internal Medicine, with extensive experience evaluating and managing complex medical conditions across inpatient and outpatient settings. His career spans hospital medicine, primary care, hospice, palliative care, healthcare administration, and medicolegal evaluation.

Since 2005, Dr. Nabili has served as a hospitalist in Orange County’s premier hospital systems. Throughout his career, he has provided comprehensive medical care in emergency departments, medical-surgical units, telemetry units, intensive care settings, and post-acute care environments. He has also practiced outpatient internal medicine, including service in a Federally Qualified Health Center, focusing on quality improvement, care coordination, and patient-centered care.

In addition to his hospitalist practice, Dr. Nabili currently serves as Hospice Medical Director for VITAS Healthcare, providing clinical and administrative oversight, interdisciplinary team leadership, hospice eligibility determinations, and quality assurance activities. He previously played a key leadership role in developing and expanding palliative care programs, including strategic planning, payor contract negotiations, and supervision of advanced practice providers.

He also serves as a clinical preceptor for fellows in the Loma Linda University Hospice and Palliative Medicine Fellowship Program and continues to educate community physicians, nurse practitioners, and medical residents in the field of hospice and palliative care.

Since October 2025, Dr. Nabili has served as an Internal Medicine Qualified Medical Evaluator (QME) in the State of California. In addition to his clinical practice, he serves as a medical expert witness and has extensive experience in medical case review. He previously served as a Medical Consultant and Medical Expert Reviewer for the Medical Board of California and participated in hospital peer review and quality committees for many years. His areas of expertise include standards of care, internal medicine, complex medical case analysis, causation, apportionment, permanent and stationary evaluations, impairment and disability assessment, medical necessity, healthcare delivery systems, hospice and palliative medicine, and end-of-life care.

Dr. Nabili earned his Doctor of Medicine from St. George’s University School of Medicine and his Master of Public Health from the University of California, Los Angeles. He completed his Internal Medicine residency at Alameda County Medical Center–Highland Hospital, an affiliate of the University of California, San Francisco. He currently holds active medical licenses in California and Nevada and serves as Chief Editor of Palliative Care for Medscape.
